Summary/Objective

The Senior Services Coordinator coordinates services for senior clients which supports higher quality of life. The Senior Services Coordinator must be knowledgeable of the service needs of seniors.

Essential Functions

The Senior Services Coordinator specializes in coordinating services for the senior population and families. The Senior Services Coordinator is responsible for providing requested services to senior clients as needed.

  1. Caseload includes households with seniors over the age of 62+ in the E&ES FW assigned services areas
  2. Conduct outreach via various outreach methods (in-Person, telephone, email/mail correspondence, social media, etc.) to seniors
  3. Complete Quality of Life Assessments and Home Visits and any other CHA or E&ES designated paperwork for seniors
  4. Perform direct services related to seniors
  5. Participate in Multidisciplinary Case Staffings conducted by the Program Managers Provides briefings to staff on senior-related issues and resources
  6. Attend CHA senior-related meetings; maintain necessary contact and follow up with relevant parties from CHA; maintain a positive working relationship with CHA
  7. Attend trainings related to senior issues
  8. Collaborate with E&ES and CHA partner agencies who offer services for seniors
  9. Link clients with transit support and makes internal and external referrals to E&ES partners and community providers, including, but not limited to: Energy Assistance, Financial, Housing, Support Services, Taxes, Transportation, Health, etc.
  10. Develop resources and partnerships for Quality-of-Life services
  11. Assist in maintaining master Frail List and performs well-being checks
  12. Organize and facilitate events for seniors under the direction of the Program Manager

Skills and Experience

Have experience related to senior services

  • Excellent presentation and communication skills
  • Proficiency with Microsoft Office Suite
  • Experience working with diverse, at risk, and vulnerable populations
  • Understanding of family-centered coaching
  • Work well independently and as a team member
  • Strong communication skills that lend to building effective partnerships with related service providers
  • Knowledgeable of the community resources that offer self-sufficiency, youth empowerment services, and quality of life services
  • Self-motivated and be able to effectively market FamilyWorks services to clients
  • Extensive outreach experience and proven high engagement with Chicago Housing Authority residents
  • Proven track record in delivering on performance goals and metrics

Physical Demands

The physical demands described here are a representative of those that must be met by an employee to successfully perform the essential functions of this job. This is a field role with a need to perform clerical and administrative duties in a physical office. Role requires extensive amount of mobility.

Hours of Work

Days and hours of work are Monday through Friday, 8:30 a.m. to 5:00 p.m. Position requires the ability to work on evenings and weekends as needed.

Reporting Relationships

This position reports to the Program Manager and works in collaboration with E&ES staff. Will also maintain positive relationships with Chicago Housing Authority and partner agencies.

Education

High School Diploma or Bachelor’s Degree from an accredited College/University or related experience
