Summary/Objective

The Community Outreach Specialist is responsible for providing Family Centered Coaching services and assisting clients to ensure lease compliance and housing stability.

Essential Functions

The Community Outreach Specialist is responsible for working with Chicago Housing Authorities families to assess needs, assist with goals setting and developing action plans. The Community Outreach Specialist will use Family Centered Coaching to identify strengths, encourage progress on goals and coordinate needed services. The Community Outreach Specialist is responsible for following up on housing related referrals from property managers, Chicago Housing Authority, and E&ES staff as well as assisting self-referred clients with housing services. In conjunction with E&ES staff, the Community Outreach Specialist works closely with clients to address housing needs.

1. Utilize various outreach methods (in-person, telephone, email/mail correspondence, etc.) to market FamilyWorks services and engage families in services

2. Refer families to external services or internal services based on service need and request

3. Assist families with the development and achievement of identified goals and assist them with the resolution of any identified barriers to goals. Work with families to achieve outcomes in various service areas to foster economic independence, academic achievement, increase earning power, enhancing stability and quality of life

4. Provide follow up to support families with goal progression and achievement

5. Manage lease compliance referral process for assigned site(s) and address housing related issues

6. Offer services and referrals to help families meet eligibility criteria for preferred housing and resolve lease compliance violations.

7. Log, track, and address lease compliance issues with clients. Determine what services are needed and map out plan to become compliant

8. Link families with transit support and provides referrals to community service providers and E&ES partners when appropriate

9. Participate in case staffing and supervisions conducted by the Program Manager

10. Represent FamilyWorks for agency and program in community meetings and events

Skills and Experience

· Minimum 2-3 years of experience in human or social services

· Excellent presentation and communication skills

· Proficiency with Microsoft Office Suite

· Experience working with diverse, at risk, and vulnerable populations

· Understanding of family-centered coaching

· Work well independently and as a team member

· Strong communication skills that lend to building effective partnerships with related service providers

· Knowledgeable of the community resources that offer self-sufficiency, youth empowerment services, and quality of life services

· Self-motivated and be able to effectively market FamilyWorks services to clients

· Extensive outreach experience and proven high engagement with Chicago Housing Authority residents

· Proven track record in delivering on performance goals and metrics

Physical Demands

The physical demands described here are a representative of those that must be met by an employee to successfully perform the essential functions of this job. This is a field role with a need to perform clerical and administrative duties in a physical office. Role requires extensive amount of mobility. Possession of a valid driver’s license, automobile insurance and reliable automobile required

Hours of Work

Days and hours of work are Monday through Friday, 8:30 a.m. to 5:00 p.m. Position requires the ability to work on evenings and weekends as needed.

Reporting Relationships

This position reports to the Program Manager and works in collaboration with E&ES staff. Will also maintain effective relationships with Chicago Housing Authority staff and partner agencies.

Education

Bachelor’s degree preferred OR 5 years related experience
